Following the distribution of home testing kits, I am writing to ask that all parents/carers and their children who are completing the testing, complete the online form below each time a test is taken. This will help us all to minimise the spread of Covid-19 in our community. The form will only take a few minutes to complete; your time is really appreciated at this challenging time.
